Yes! The
most requested dental procedures are included in
the schedule of dental benefits. There are many
dental procedures listed (over 200 )
and....allowances have been made for dental
procedures not listed.

eligible to be on my program?
Answer:
You, your
spouse and eligible dependants (all unmarried
dependant children through age 23)

Will I
receive a new membership package?
Answer:
Yes! The
complete Care-Dent Package will be sent to you.
Your new membership package will include the
Complete Dental Benefits Schedule, directories
listing participating Dentist, Pharmacies,
Optical and Vision Centers. In addition, you
will receive membership identification cards,
and a guide for using your program benefits.
How do I use Care-Dent?
Dental Benefits:
Telephone your selected primary care dentist or the
dental specialist from our list for an appointment.
Tell
the dental office you are a Care-Dent member.
When
you visit your Care-Dent Dental Provider, a
co-payment may be required - if so, simply pay the
discounted fee to your dentist's office.
Vision Benefits:
For
Vision services, simply phone the PNVS Eye care
Provider of your choice for an appointment. Pay the
discounted fee for services directly to the Eye care
Provider.
Prescription Benefits:
To
use your Pharmacy services, simply take your
prescription to the Care-Dent Pharmacy Provider of
your choice in order to receive your discount. Pay
your discounted fees directly to the pharmacy.
Dental information
not listed in the Program Schedule:
Any dental
procedure not listed in the Benefits Schedule and is
performed by your primary care or general dentist
will be discounted 20% of Dentist's usual and
customary rate (UCR)
Additional
fees may be charged by the Dentist for dental
procedures that presents unusual circumstances and
difficulties.
Services Provided
by the Specialists:
Endodontics,
Orthodontics, Periodontics, Oral Surgery,
Pedodonitcs (all available depending upon services
area and availability).
You will
receive a 25% discounted savings from the
Specialist's usual and customary rate (UCR) when
dental services are performed. You may be referred
by the General Dentist, or you may refer yourself
directly to the Specialists when in need of
Specialists services.
