Care-Dent, Inc. can not guarantee the continued
participation of any provider as listed in our program
and or programs and displayed within our web site. If a
dental provider leaves the program you will need to
select another dentist. The dollar amount specified next
to each procedure may not be the only cost incurred for
any given treatment. Treatment may require additional
lab fees. Check with your provider for current lab fees,
if applicable.
